This study finds how in Burkina Faso the formal removal of user fees for tuberculosis treatment (DOTS) does not always protect households from catastrophic health expenditure. Reasons for this were found to be due to a policy gap (additional services were needed but not covered under the policy and had to be paid out-of-pocket), and to health system failures, mainly due to inefficient patient management forcing patients to make additional direct and indirect costs as well. The paper calls for improved evaluation of the implementation of this kind of policies and for proper policy design taking into account the national context and socio-economic reality of the people concerned.

This paper analyzes vertical and horizontal equity as well as eventual catastrophic health expenditure as a result of health insurance contributions payable to the Ghanaian National Health Insurance Scheme (NHIS). The NHIS aims to achieve universal health coverage and to protect the population of Ghana against the financial risks related to health care needs. The study finds that health insurance premiums are regressive (vertically inequitable), and that horizontal inequity is relatively high in Ghana implying that similar income groups pay different premiums and in particular some lower income groups pay higher premiums than some higher income groups. For a small proportion of the poor, the health insurance premium was found to be catastrophic. Recommendations on how the scheme could improve this situation are provided.
